The Queen's Own Rifles of Canada Memorial and Book of Remembrance were both erected by The Queen's Own Rifles Association and dedicated on March 13, 1932. The Book of Remembrance includes the names of all members of the regiment who died in the service of their country – in combat, and in peacetime. It is kept in a special case in a memorial shrine at the back of the main chancel (west side) of St. Paul’s Bloor Street – the Regimental Church.

The Book of Remembrance is taken out at each service at which the regiment is present, by a special party of senior officers and non-commissioned officers, and escorted to the front. It is presented to the Commanding Officer, who in turn presents it to the Rector who places it on the High Table (Altar) during the service. The process is reversed at the end of the service to return it to its place of honour.

There are two Books in two locations. The book with the plain green cover that is chained to its own stone plinth to the left of the memorial is available for anyone to look through. The Book of Remembrance is locked in the glass and brass case on the memorial.
