Overview:

The objective of this project was to stabilize Veterans through psychosocial support and referrals, and to direct Veterans to appropriate resources in order to optimize the chances of a full recovery.

Project Goals:

The main goal of this project was to assist 15-20 Veterans. For each Veteran, an evolving psychosocial file and personalized intervention plan were established.

Project Activities:

The organization worked to determine, through a classification system, the needs of each Veteran encountered. A triage was completed to prioritize each request. With the active participation of the clinical coordinator, the case manager was able to apply rehabilitation and follow-up measures to the file.

Project Results:

Of 23 Veterans who were referred to the program by partner organizations, the organization successfully assisted 12 Veterans in finding long-term housing.
