This plaque was erected in the Church of the Epiphany in honour of Sergeant-Major Frank Gibson Addison. The Church of the Epiphany was located at 1515 Queen Street West until it was sold on 11 June 1983 to the Our Lady of Lebanon Parish. Frank reverted to the rank of private in order to proceed to France.
