His full name is Thomas Alfred Chapman.

On 11 June 1942, at 4.55 am, about 20 miles (32 km) west of Ship Shoal Buoy, in the Gulf of Mexico, she was heading unescorted towards Houston, Texas, USA, when she was torpedoed by U-158. One crew member, Thomas Alfred Chapman, was killed. The SS Midshipman, a shrimp boat, recovered 35 survivors and landed them in Morgan City, Louisiana, USA, on the same day, while the FV 40 Fathoms recovered 23 and landed them in the same place.
