On 30 October 1942, Bullmouth was firing from astern with convoy SL-25 when, at 00:21 am , she was damaged by a torpedo launched from U-409 about 100 miles (160 km) from Madeira Island, Portugal. At 1:40 am, U-659 fired two torpedoes at her, sinking her in position 33°20'N/18°25'W. Her captain, 44 crew members and 5 gunners lost their lives. The six survivors managed to reach the island of Bugio, Portugal.
