Chalmers Presbyterian Church dedicated this memorial plaque to those from its Sunday School who died because of their service in the First World War. This wooden plaque takes the form of a shield and has thirty maple leaves each with the name of a man who died. Plaques like this were part of the Peace Thank-offering and Forward Movement with which the Presbyterian Church marked the end of the war.
The church sanctuary at 1266 Dundas Street W was destroyed by fire in 1945 and was rebuilt. In 1978, the congregation merged with St. Paul's Dovercourt Presbyterian Church and the church on Dundas Street was demolished in the 1980s.
