Son of Daniel Francis Simmons and Agnes Stapleton of 162, Falkner Street, Liverpool, England. Brother of Private Raphael Simmons, regimental number 2103486, Army Service Corps - Mechanical Transport, British Army, who fought in France in the First World War. He survived the war.

On 20 October 1917, she was en route from Milford Haven, England, to Plymouth, England, when she struck a mine in the St George’s Channel laid by UC-51 and sank 2 miles (3 km) west of St. Govan's Head, Pembrokeshire, Wales, position 51°35'N/04°59'W. The crew took refuge in the lifeboats when one of them capsized, drowning 7 of the 22 people on board.
