Baptized Joseph-Donat-Frédéric Bernaquez. Son of Napoléon (alias Paul) Bernaquez and Mathilde Dumont (deceased in 1905), of Trois-Rivières, Mauricie, Québec. His father remarried to Virginie Vézina in 1910 and he died in 1915, one month after his son went overseas. Donat then named his sister, Marie-Blanche-Aurore (wife of Rémi Hamelin) as next of kin.

He was killed in action on 9 April 1917 on Vimy Ridge, France, in the area of the Souchez and Scarpe rivers.
